What is BIZUSIZO?
BIZUSIZO is a digital implementation of structured patient streaming for South African public clinics. It uses WhatsApp, a self-service kiosk, and a clinic dashboard to triage patients before they arrive, route them to the correct service stream, and give nurses the tools to manage queues effectively — in all 11 official languages.
Does BIZUSIZO diagnose or prescribe?
No. BIZUSIZO provides healthcare guidance, not diagnosis or prescription. We are positioned carefully under SAHPRA regulations to remain outside restrictive medical device classifications. Our role is to help patients understand their urgency level and find the right facility — the clinical decision always rests with a nurse or doctor.
How does the triage work?
Patients describe their symptoms through WhatsApp or a kiosk tablet. Our system uses a combination of a deterministic rules engine (for immediate emergency detection) and AI-assisted clinical reasoning (aligned with the South African Triage Scale) to classify urgency into four colour-coded levels: RED (emergency), ORANGE (very urgent), YELLOW (urgent), and GREEN (routine). Risk factors like HIV, diabetes, hypertension, and pregnancy are assessed and can only raise — never lower — the triage level.
What happens in an emergency?
RED cases are detected immediately by our rules engine — before AI processing even begins. The patient is told to call 10177 (ambulance) or go to the nearest emergency unit immediately. The clinic dashboard is alerted so staff can prepare. A human healthcare worker is always in the loop for critical cases.
What languages are supported?
All 11 official South African languages: English, isiZulu, isiXhosa, Afrikaans, Sesotho, Sepedi, Setswana, Xitsonga, siSwati, Tshivenda, and isiNdebele. Our system also understands code-switching and township medical terminology.
Is my data safe?
Yes. BIZUSIZO is fully POPIA compliant. We collect consent before any health data is gathered. Your phone number is hashed using SHA-256 — we never store it in plain text. All data is encrypted and a full audit trail is maintained. We do not sell or share patient data.
Do I need to download an app?
No. BIZUSIZO works entirely through WhatsApp — no app download, no registration, no account creation. Just send "Hi" to our WhatsApp number. If you don't have a smartphone, you can use the self-service kiosk tablet at participating clinics, or the reception nurse can register you directly.
What is the nurse's role?
The nurse always has the final word. When a patient arrives, the nurse reviews the AI triage on the clinic dashboard and can Agree or Disagree with the classification. Disagreements are logged for continuous improvement. Nurses can also pause, resume, escalate, and refer patients through the dashboard.
How does BIZUSIZO align with the NHI?
The National Health Insurance Act 20 of 2023 mandates strengthened primary healthcare, improved referral pathways, and dedicated digital health infrastructure. BIZUSIZO directly operationalises these mandates — our structured patient streaming, FHIR-compliant data architecture, and DHIS2 integration readiness are built specifically for NHI alignment.
What about chronic medication patients?
Stable chronic patients (e.g., collecting routine hypertension or diabetes medication) can bypass full triage and be routed directly to the chronic care stream — reducing the 70% of clinic queues caused by routine medication collection. Our architecture is designed for future CCMDD integration, routing patients to external pickup points.
How can my clinic or organisation get involved?
